Think you or your loved one need Long-Term Care? (Fraser Health) [English] Permalink Public
Long-Term care funded by Fraser Health is a service for adult/seniors with complex medical and/or physical conditions with or without dementia, who do not have the supports in their home to manage and require 24-hour access to nursing care. Long-Term care is not a housing option. It should only be considered after trialing all other resources while still living in your home. Fraser Health staff will work with you and your caregiver to explore options that can help you stay as independent as possible in your home. (English) Colour
